As tests and procedures and/or Immunizations are marked off as completed in the EMR while charting, those CPT codes are getting added to the Encounter Charges. Therefore, most of the time you will see the E&M code at the bottom and the lab codes, vaccine codes, etc at the top.

Also, if you delete codes, you may see gaps in the numbering.

You do not need to correct and reorder these manually.

When you mark the claim Ready to Bill, the system will automatically correct any duplicates or gaps and will reorder them to have the E&M code in Line Item 1 position. 

If you wish to see this BEFORE you mark the claim Ready to Bill, you can use the "Reorder Chgs" button from Encounters -> Charges -> Claim Info.
