How to Import a CCDA Document

It is possible for another medical provider to provide to you a specially formatted document called a CCDA (Consolidated Clinical Document Architecture). This document has several sections that cover many parts of a patient's clinical chart. 

 

Steps to import the CCDA:

1. Navigate to Document Management and select the "Browse" button to find your CCDA file (this should have an "xml" file extension)

 

 

2. The system will then recognize this as a CCDA and attempt to parse and display the Human Readable Version. If there are any errors or warnings found, you might be presented with those first. You can then simply select the "View Document" tab at the top:

 

 

3. This will display the human readable version of the CCDA with all the different sections that you can navigate to either from the Table of Contents on the left hand side or the hyperlinks at the top of the page. You should notice at the top of this import screen, there are fields that you can populate in order to simultaneously save a Care Transition record. You can also just click on the checkbox to indicate "Do not Create a Care Transition"

 

You then have 3 options: Save to Disk ,Parse to Import, or Save to Chart. 

- Save to Disk will simply save this CCDA to a location on your computer that you indicate

- Parse to Import will attempt to parse the medications, medication allergies and problems to the patient chart

- Save to Chart will save and categorize the human readable file in the Patient Documents section of the chart under the document type of CCDA

 

 

4. If you select Parse to Import or Save to Chart, you will be shown the patient search window so you can either search an exiting patient or use the New button to create a new patient at this time. 

 

 

5. Once you either enter a new patient, or select an existing one, the system will show you the medications, allergies, and problems it parsed. On this screen you can use the "Reconcile" button to reconcile any duplicates. This is an example of a new patient, therefore on the left, there are no existing medications, allergies, or problems displayed:

 

 

6. Once the reconciliation is done, you may see "No Duplicates Found" above each section and there should now also be a new button labeled "Import Data to"

At this point, you also have the option of selecting/un-selecting each section:

 

 

7. Once it imports, you will get a success message. Note the only allergies that will import will be medication allergies. 

 

 

8. From Patient Documents you will be able to see this full human readable document under the CCDA document type:
