How can I enter Medical History without an Encounter?

There is not a way to enter the history data without an encounter.

This includes the following screens:

- Past Medical History

- Social History

- Family History

- Past Surgical History

- Hospitalizations

- Functional Status

- Cognitive Status

- any custom Medical History screen

Only Medications, Allergies and Immunizations are allowed to be entered without an encounter.  We use encounters to track any changes/updates to history throughout the months/years you are seeing the patient.

You can, however, add an encounter with the appointment type = History.

Go to Encounters, then use the "New Encounter" button on the right side. Select History as the appointment type. This will create an appointment, but a non-face-to-face one that will not be counted in most reports.
