If the dx was added in error, you will need to find out when the dx was first added reopen encounter and delete from assessment. You will need to delete the dx from every encounter it was added to.
If the dx was added correctly and it is just that the patient no longer has the problem, then you will need to just resolve the dx.
Go to Pt chart > Summary of care> Chronic/Problem list
Find DX change status to resolved.
