GOALS

 

ReLiMed EMR’s Goal module allows you to enter Goals, Objectives, Interventions. Set frequency and target dates for each objective. Track them over time and when resolved, change the status to Resolved.

 

Adding Goals/Objectives

During the Initial or subsequent encounter, Goals can be set up for a patient. ReLiMed Goals module allows you to select a Goal from a preset list or add a custom goal.

 

•  Go to Patient Chart -> Encounters -> Goals
•  Click on Add Goals

 

 

In the Add Goals popup

 

•  To Add a New Goal to the patient

           o Select an item from the preset goals list by selecting the row

           o Click on Select button

           OR

           o You can enter a custom goal for the patient by typing it up in the Custom Goal edit box

           o Then click on Add Custom button

 

• You can also search for a goal in the list by typing up a word or a phrase

           o Then click on Find

           o Click Clear to clear the search phrase and view all the goals

 

 

 

•  A goal text can be edited once added to patients’ chart to further customize it
•  If the provider has added a diagnosis (problems) to the encounter, they can select the diagnosis that is appropriate for the added goal by clicking on the field and selecting it from the dropdown list

 

• After the Goal is added, they can click on the Add Objective to add the Objective(s) for the goal
• With objective, as in adding goals, provider can

            o Select an item from the preset objectives list by selecting the row

            o Click on Select button

            OR

            o You can enter a custom goal for the patient by typing it up in the Custom objective edit box

            o Then click on Add Custom button

 

•  Objectives associated with a goal are listed by default. Click Show all to view all objectives that are available in the preset list

 

•  Once the objectives are added to the goals, users can

        o Edit and further customize the objective

        o Free text interventions

        o Free text frequency

        o Set Target Date

        o These Objectives go in as 'New Status'.

 

 

Importing Goals/Objectives

Providers can import previously set up goals for the patient in the subsequent visits. They can import all the goals and objectives or just a subset of them that they plan to address during that encounter.

 

To Import goal

 

• Go to Patient Chart -> Encounters -> Goals
• Click on Import Goals

 

• This pop up first will only show you the 'New' status objectives.
• To find one you have edited since originally creating the goal and objective you will select the box 'Show all goals for this patient (*you may see duplicates)
• Selected Goals/Objectives are imported into the current encounter
• They get added with a status of 'In progress'
• Providers can add new goals and objections as well along with working on existing ones.

 

 

Change Status

After working on certain goals/objectives, providers can change status to mark them as ‘Met’ or other relevant status.

 

Users can click on Change Status and select the appropriate status in the popup and save

 

 

 

Adding Goals and Objectives List in the system (Admin)

• Go to Admin -> Lists -> List Item Entry
• For Goals -> Select Goals and Add/Edit from this screen
• For Objectives -> Select Objectives and Add/Edit from this screen
• The system will ask you to pick a goal the objective is typically added for
